Indications / Uses of RENITE-5 XL Enalapril Maleate 5mg Tablet

Enalapril is an ACE inhibitor that lowers blood pressure by widening blood vessels, making it easier for the heart to pump blood.
Enalapril is used to:
  • Treat high blood pressure (hypertension)
  • Treat heart failure, reducing hospital visits and potentially increasing lifespan
  • Prevent symptoms of heart failure, such as shortness of breath, fatigue after mild activity, and swelling in the ankles and feet.

What are the side-effects of RENITE-5 XL Enalapril Maleate 5mg Tablet?

Like all medications, Enalapril can cause side effects, though not everyone experiences them. Seek immediate medical attention if you experience:
  • Itching, raised red rash (hives)
  • Shortness of breath or wheezing
  • Swelling of hands, feet, face, eyes, mouth, lips, tongue, or throat, which may cause difficulty breathing or swallowing
These may be signs of a serious allergic reaction.
If you experience any of the following serious side effects, contact your doctor immediately:
  • Complex side effects: Fever, inflammation of blood vessels, pain and inflammation of muscles or joints, blood disorders (detected by blood tests), skin issues (rash, hypersensitivity to sunlight).
Other side effects include:
Very common (affecting more than 1 in 10 people):
  • Blurred vision
  • Dizziness
  • Cough
  • Nausea
  • Weakness
Common (affecting up to 1 in 10 people):
  • Headache
  • Depression
  • Low blood pressure (causing light-headedness)
  • Fainting (syncope)
  • Heart rhythm changes (fast heartbeat, chest pain, angina)
  • Difficulty breathing
  • Diarrhea
  • Abdominal pain
  • Change in taste
  • Rash
  • Fatigue
  • Allergic reactions (swelling of face, lips, tongue, or throat, difficulty swallowing or breathing)
  • Increased blood potassium and creatinine levels (detected by blood tests).

Dosage / Direction for Use of RENITE-5 XL Enalapril Maleate 5mg Tablet

Adults:
  • High Blood Pressure (Hypertension):
    • Initial dose: 5 mg to 20 mg daily.
    • Usual dose: 20 mg daily.
    • Max dose: 40 mg daily.
    • Adjustments: Dose tailored by your doctor based on blood pressure and other conditions. If on high doses of diuretics, stop them for 2-3 days before starting Enalapril.
  • Heart Failure:
    • Starting dose: 2.5 mg daily.
    • Usual dose: 20 mg daily (one or two doses).
    • Max dose: 40 mg daily, split into two 20 mg doses.
  • Kidney Problems: Dose adjusted based on kidney function. If on dialysis, dose may vary daily.
  • Elderly: Dose determined by your doctor, based on kidney function.
Children:
  • Limited use for high blood pressure.
  • If the child can swallow tablets, dose depends on weight and blood pressure response.
  • Children with kidney problems should not take Enalapril.
  • Not recommended for very young babies.
How to Take Enalapril:
  • Take with or without food, at the same time each day.
  • Use the blister pack to track doses.
  • The first dose may cause dizziness; this usually decreases with future doses.
  • Your doctor will monitor blood pressure and conduct blood tests. Contact your doctor or pharmacist if you feel unwell.

Contraindications

Do not take Enalapril if you:
  • Are allergic to enalapril maleate, any other ACE inhibitors, or any of the ingredients in the medicine.
  • Have a family history of allergies to ACE inhibitors, or have ever experienced angioedema (swelling of the face, lips, mouth, tongue, or throat) with an unknown cause or inherited tendency.
  • Have diabetes or impaired kidney function and are taking a blood pressure medicine containing aliskiren.
  • Are more than 3 months pregnant (avoid in early pregnancy as well).
  • Are currently taking or have taken sacubitril/valsartan for chronic heart failure, as this increases the risk of angioedema.
If any of the above apply to you, do not take Enalapril. If unsure, consult your doctor.

Special Precautions

Talk to your doctor or pharmacist before taking Enalapril if you:
  • Have low blood pressure (may cause dizziness or fainting, especially when standing).
  • Have a brain blood vessel condition.
  • Have a heart problem.
  • Have kidney problems (including kidney transplantation) or are on a salt-restricted diet, taking potassium supplements or potassium-sparing agents, or other drugs that can raise potassium levels (e.g., heparin, trimethoprim-containing products).
  • Have a liver problem.
  • Have a blood disorder (e.g., low white blood cells, low platelets, or anemia).
  • Have ever had an allergic reaction, angioedema (swelling of face, lips, tongue, throat), or angioneurotic oedema.
  • Are black (increased risk of angioedema with ACE inhibitors).
  • Are undergoing dialysis.
  • Have had severe vomiting or diarrhea recently.
  • Have diabetes (monitor blood glucose and potassium levels).
  • Have collagen vascular disease (e.g., lupus, rheumatoid arthritis) or take medications that suppress the immune system (e.g., allopurinol, procainamide).
  • Are taking racecadotril, medications for organ transplant rejection (e.g., temsirolimus, sirolimus), or vildagliptin (for diabetes), as these can increase the risk of angioedema.
  • Are taking other blood pressure medicines, especially angiotensin II receptor blockers (ARBs) or aliskiren (particularly if you have diabetes-related kidney problems).
  • Are pregnant or planning to become pregnant (not recommended in early pregnancy and must not be used after 3 months of pregnancy).
Note: Enalapril is less effective at lowering blood pressure in black patients.
